Home Care Dependable support at home — personal care, companionship and help with everyday tasks — so people can live… Hospital Transition Coordinated resources, follow-up and home support for a safe, steady transition after a hospital stay. Hospice Support Compassionate coordination and support for individuals and families during hospice care.

Housing & Living

Independent Living Resources, planning and services that help people live independently and remove the practical barriers in the way. Housing Assistance Guidance and connections that help individuals and families navigate housing needs and reach the organizations that can assist. Care Home Help understanding care home options and finding a setting that fits a loved one’s needs and circumstances. Adult Day Program Structured, supportive day programs for adults who benefit from engagement, supervision and connection. Assisted Living Help exploring assisted living options and understanding what each setting offers.

Work & Community

Career Employment Employment resources, opportunities and support that help a job grow into a stable career. Food Bank Connections to food resources and the local organizations that provide them, for individuals and families. Employee Program Resources, benefits and assistance for the employees who carry out community care.

Access & Coordination
